Do you take insurance?

I do not take insurance.
As such, appointments are self-pay and payment is required upfront.
You can submit payment through a Superbill to your insurance company for possible reimbursement after all appointments are completed.
You can also use your HSA card, if you have one.

How does self-pay work?

I charge a flat fee upfront (after the intake appointment) based on the evaluation provided. A refund for appointments that are not conducted is available if you cancel at least 72 hours prior to appointment (less any appointments already conducted).
I am happy to provide a superbill that you can submit to your insurance company for possible reimbursement. This is given after all appointments are completed, as insurance will not reimburse for appointments not yet conducted.

What about insurance?

It is your responsibility to understand your insurance coverage as it pertains to evaluation services.

This responsibility includes:

Contacting your insurance carrier to verify your insurance coverage with respect to out-of-network evaluation services. It is recommended that you call your insurance company prior to the first appointment so that you know what they may reimburse you.
Knowing what your deductible and copay amounts for various services are (e.g., office visit copay), and where you are at within your deductible cycle. This information will help determine what your responsibility may be for services.
For your reference, the billing codes are: 90791, 96136, 96137, 96132, and 96133.

What do you charge?

Evaluations range from $1900-$5000, depending on the type of evaluation (e.g., IQ only, psychoeducational, comprehensive, etc.).

For example, a comprehensive evaluation, which is 5 hours and tests all domains of cognitive skills, is $5000 (flat fee). This is the most common type of evaluation.
The evaluation type is decided at the intake appointment after I have heard the concerns in detail, but I can give an estimate prior.
You can decide at the intake if you would like to move forward. If you choose not to move forward, you will only have to pay for the intake appointment ($340).
